myForestLake Customer Portal

The following terms and conditions supplement the General Conditions of Use described above.

myForestLake Customer Portal is Forest Lake’s internet-based customer service facility accessible by its Policy Owners. They may apply for enrollment to myForestLake Customer Portal over the counter, at selected Forest Lake branch offices, and through online registration.

If you are a Policy Owner of myForestLake and decide to enroll in Forest Lake’s myForestLake Customer Portalyou will be required to enter an electronic identification which consists of a username, password, and other security codes. As a Policy Owner, you may access your policy information and/or may perform policy servicing transactions with Forest Lake upon submission of your updated personal information or any information related to the specific policy servicing transaction you will perform. However, as insured or beneficiary, you will have limited access to your Customer Portal account.

Forest Lake implements different tools, methods, and processes to protect your account information and online transactions. We employ security products, such as intrusion detection systems and firewalls, and other internal controls which are meant to safeguard, physically and logically, all our servers and information systems, including the data stored in these systems. Access to myForestLake Customer Portal through the Forest Lake website (https://www.forestlakememorialparks.com/) is secured which enables SSL for end-to-end encryption.

By using this website to access your policy, you consent to the electronic transmission of your personal information and any other information you have provided related to the specific policy servicing transaction.

It is agreed that your initial use of your electronic identifications in accessing Customer Portal is the legal equivalent to your written acceptance to the General Conditions of Use and that your identity as an authorized user of the Customer Portal shall thereafter be conclusively established by the presentment to us when required of said username, password and security codes.

Your Responsibility to Prevent Unauthorized Access to Your Accounts

You undertake to protect your username, password and security codes, and to be solely responsible for the confidentiality and use of such electronic identifications. You accept full responsibility for all transactions entered through your account.

You agree to immediately notify us if you believe your username, password and security codes have been compromised, or if you become aware that these electronic identifications are being used in an unauthorized manner or if you receive any confirmation of a transaction that you did not enter, or if your received confirmation or a report containing conflicting or inconsistent information regarding your policy.

Failure to notify us of one of the above conditions will release us and our employees and associates from any responsibility or liability to you or any other person or entity resulting from the management or loss of any transaction.

We reserve the right to block electronic access to your account at any time. We further reserve the right to modify the information contained on this website or modify the availability of any transactions at any time. We may immediately and without notice terminate your access to your account information if you violate the conditions of use stated herein or if you endanger the use of the services provided through this website.

We have in our custody the information you have indicated in your buyer application form and the information about your policy contract. myForestLake respects the privacy of your personal and policy information.

Moreover, we are very careful to preserve the fiduciary nature of our relationship with you. While the very nature of our business sometimes requires that we collect or share certain information about you with others, we make sure that you consent to our handling and protection of your personal and policy information.

We may collect non-public personal information about you from

- Applications or other forms, interviews, or by other means;

Declaration

I declare that I have read, understood, and agree with the above Terms and Conditions. I also hereby certify that all the personal data contained herein are true and correct.

I also understand that as a memorial park institution, Forest Lake is subject to existing and future government regulations. I therefore agree to be bound by all applicable domestic and international laws in relation to any matter including, but not limited to, anti-money laundering, tax monitoring and data privacy.

In this connection, I authorize Forest Lake to process my personal and sensitive personal information including but not limited to its collection, use, retention, destruction or sharing to our subsidiaries, affiliates, agents and authorized third parties for any legitimate purpose including, but not limited to, underwriting and administration of policies, marketing and promotion of products, market research, data analytics and automated processing systems, internal and external audit.

I hold Forest Lake free and harmless from any liability that may arise from any collection, use, destruction or sharing of said information.
